The company has set the list price for a 30-day supply at USD $39,800. This figure implies an annual acquisition cost exceeding USD $477,000, before discounts, negotiations, or coverage that may be applied by each insurer. The price was above the USD $25,000 or USD $30,000 that some analysts had anticipated.

A Launch with Extraordinary Expectations

The reaction from analysts was immediate following the approval and pricing details. Cory Kasimov, an analyst at Evercore ISI, wrote to his clients that Rasonque had the necessary elements to become one of the fastest oncology launches in history, or even the fastest.

In a second note published after Revolution's conference call, Kasimov described the drug's introduction as transformative and historic. His firm raised the sales forecast for Rasonque in pancreatic cancer to USD $2.4 billion by 2027 and to USD $15.1 billion by 2034, figures calculated solely for that disease.

Evercore ISI also considers that Rasonque could gain approvals to treat other types of cancer, an expansion that would substantially change the potential market size. Under that scenario, the drug could reach maximum annual revenues of USD $20.8 billion and enter the competition for the world's best-selling pharmaceuticals.

The projections do not solely depend on Revolution charging more than initially expected. They also reflect the possibility that the company could leverage an indication with few therapeutic alternatives, a relatively flexible label, and accumulated demand among patients with metastatic pancreatic tumors. These are estimates, not guaranteed financial results.

What the FDA Authorization Allows

The FDA authorized Rasonque for patients who have already received systemic therapy for metastatic pancreatic adenocarcinoma or who are not candidates for a systemic therapy that combines multiple drugs. The conjunction used in the label is relevant because, according to analysts, it gives doctors some leeway to use the drug as an initial treatment in selected cases.

Some insurers may attempt to restrict that early use due to the product's cost, especially while there is no specific approval for first-line treatment. Revolution is moving forward to formally expand the use of Rasonque in first-line treatment and is currently conducting a Phase 3 trial with this goal. An eventual approval at this stage would allow the company to compete for a broader patient population and reduce uncertainty regarding doctors' access to the medication from the start of treatment.

The results released after the Phase 3 RASolute 302 trial showed a median survival of 13.2 months with Rasonque, compared to 6.7 months with chemotherapy. Revolution also reported a 60% reduction in the risk of death. These data help explain the enthusiasm surrounding the drug, although they do not eliminate the need to evaluate its risks, benefits, and limitations for each patient.

The label thus becomes a commercial asset in addition to a regulatory one. Its wording can facilitate clinical decisions for certain patients, although the price will remain a point of discussion among hospitals, insurers, and health systems that must absorb an annual cost exceeding USD $477,000 per person.

Immediate Access and Challenges for Patients

Revolution has made the pill available immediately and allocated resources for the commercial launch, a speed that seeks to capitalize on the momentum generated by the approval. The company also created the (ON)Path program, designed to help patients manage insurance coverage, locate financial assistance, and handle the side effects associated with the treatment.

The program will be important because the list price does not necessarily represent the final out-of-pocket expense for each patient, but it does set a high reference point for negotiations with payers. In practice, Revolution's ability to coordinate authorizations, financial support, and medical follow-up will influence how quickly potential demand translates into treatments actually initiated.

Clinical enthusiasm coexists with significant side effects, an aspect that the expansion of the drug cannot overlook.
